Short answer

The App Store doesn't let anyone charge an arbitrary amount - Apple only allows fixed prices for in-app purchases. So Zero Balance combines a few small items to get as close to your leftover as it can, with the least overshoot.

There is no way for any app to bill an arbitrary sum through the App Store. Apple only offers a fixed set of in-app purchase price points, so a developer cannot create a one-off charge for, say, the exact 46 left on your account. Zero Balance works around that the only way possible: it treats those fixed prices like coins and assembles a combination that adds up as close to your leftover as it can. The more price points there are to work with, the smaller the overshoot - which is why the app spreads across several small tiers instead of relying on one or two.
